Muscular Dystrophy Association (MDA)
MDA will pay up to $2,000 (100%) towards the cost of a speech device, like the Communicator 2000 shown here, if you meet the following 4 conditions:
� Medical Condition:
You must have a serious speech disorder resulting from Muscular Dystrophy, Amyotrophic Lateral Sclerosis (ALS) also known as �Lou Gehrig's Disease�, or any condition listed at http://www.mdausa.org/disease/
� Speech Evaluation Report:
Have a certified SLP (speech language pathologist) complete a speech evaluation/report that states you require a "speech generating device".
� Physician Prescription:
Visit an MDA clinic and obtain a physician's prescription for a "speech generating device". Local MDA clinics are listed at http://www.mdausa.org/clinics/alsserv.html
� Health Insurance:
You have no health insurance coverage or you have been denied coverage for a "speech generating device" by your health insurance provider.

The Medicare compliant (dedicated version) Communicator 2000 costs $4,995. Medicare will pay 80% of the cost of this version.
These units qualify under the new Medicare Rules governing "SGD - Speech Generating Devices". They are pre-configured to meet the new Medicare standards effective July 1, 2001.
*** Medicare Funded SGDs (E2510): In accordance with Medicare rules, this unit can be shipped as a "dedicated" speech device with only the speech software activated. In order to gain access to Windows, customers can purchase a MS Windows license at a cost of $25 which is NOT covered by Medicare.
